DePaul Transitional Care Unit
Located within the walls of DePaul Medical Center, the Transitional Care Unit at DePaul is a short-term skilled nursing facility dedicated to helping patients in need of specialized rehabilitation services.
Patients admitted to the Transitional Care Unit at DePaul will benefit from the center's unique care model, which combined the security and resources available in a hospital setting with the coordinated, interdisciplinary team approach of comprehensive rehabilitation. This blended approach allows us to provide medical and physical rehabilitation to high-acuity patients who may require prompt access to physician specialists, diagnostic services, or emergency care.
Patients in our center benefit from medical management, physical rehabilitation services, or a combination of both. Treatment plans are tailored to reflect the needs of each individual patient. Some of the services most commonly offered include:
- IV therapies, including antibiotic administration
- Skin and wound care
- Ostomy care and education
- Diabetes care and education in self management
- Infectious disease management
- Pain management
Eligibility
When patients are referred to the Transitional Care Unit, they are considered by their physicians to be medically stable, have the potential to improve, and agree to participate in their recovery process. Prior to admission each patient is carefully assessed to determine whether his or her condition can benefit from services offered. This assessment is performed by a qualified Transition Care Unit team member.
Insurance
As a licensed skilled nursing unit, patients utilize their skilled nursing benefits to defray the costs of treatment. Insurance coverage is based on the individual patient's condition and need for skilled and/or rehabilitative services rather than a diagnosis.
